Application for personal information disclosure, corrections, etc.
We would like to inform you of our personal data management system when our customers apply for disclosure, correction, etc. of their data.
Our disclosure and correction procedures for personal data are based on the following laws.
-  The law covering protection of personal data. (Act for Protection of Computer Processed Personal Data Held by Administrative Organs)
-  Enforcement ordinance law about protection of personal data
Application for disclosure and correction
Please fill out the prescribed forms: "Request for personal data disclosure" or "Application for personal data correction," and hand one of them in with the document covering your identification and the prescribed handling charge (money order only) when applying for a disclosure and correction.
(In case of data corrections, any documents which support the data error and correction are required.)
There are two kinds of application methods: visiting the head office, or mailing the forms. (Expenses such as transportation expenses and postage are the costumer's responsible.)
Application by telephone, FAX, E-mail or other methods apart from those mentioned above are not accepted since identification is difficult.

After receiving an application for disclosure and correction from customers, the response time is generally about 10 days from the next working day after receiving the application.
However, if the documents and handling charge are incomplete when applying, the day after everything is properly received will be counted as the received date.
Furthermore, it can be extended by a maximum 20 days for a compelling reason. In this case, we will contact the customer and tell them we require more days for investigating as well as the potential date for a response.
In case the application is not accepted due to a rational reason, the response will take 10 days from the next working day after the accepted date.
*Days until a response will be counted by calendar days except Saturdays, Sundays, national holidays, New Years and any other holiday.
An application for disclosure and correction not from the principle cannot be accepted as a general rule.
However, in case of the following situations, application by a proxy is acceptable.
-  Parents of minor (under 20-years old and single)
-  Ward of adult or guardian
(2) Voluntary proxy
・A proxy who is commissioned by letter of attorney from a principle.
As a rule, an application from family (spouse) cannot be accepted without a letter of attorney.
Applications dropped off in person will be accepted between 9:30 a.m. to 5:30 p.m. during weekdays. An application received by mail after 5:30 p.m. or on holidays shall be counted as being accepted on the next business day.
(1) Documents for identification
Required identification documents are as follows:
One copy of the following photo-identification documents issued by a public organization in Japan:
-  Driver's License
-  Passport
-  Physical disability certificate
-  Alien registration card
Two originals or copies of the following documents in case any documents cannot be obtained:
-  Inkan seal certificate (not older than 3 months from the date of issue)
-  Residence certificate (not older than 3 months from the date of issue)
-  Health insurance card
-  Pension book
-  Employee identification card
-  Student identification card
(2) Identification documents for proxy
Identification documents required from a proxy are as follows.
 Parents of minor
・ Resident's certificate (a copy showing the relationship with the person who is disclosing and correcting (if under age) and the applicant (parents of the minor). The certificate cannot be older than 3 months from the date of issue), and any identification documents listed in (1) for the proxy.
-  Guardian of an adult
・Certificate of matter to be registered (not older than 3 months from the date of issue) and any identification documents listed in (1) for the proxy.
-  Voluntary proxy
・A letter of attorney from a principle (who is applying for the disclosure and correction) and any identification documents listed in (1) for the proxy.
All documents that are produced (including a letter of attorney) will be returned to clients and will not be held.
Information concerning the disclosure and correction
The information held by us to disclose and correct should be items that are tendered by the principle as a rule.
When requesting a correction to data, documents supporting the error are required.
Response to the disclosure, correction, etc.
When we decide to act on the disclosure, correction, etc., we will inform you about the contents of the response in the prescribed forms, "Request for personal data disclosure" or "Application for personal data correction" by certified mail.
A fee for disclosure application shall be a flat 1,500 yen (including tax), and should be paid at the time of application. A money order will be the only accepted form of payment.
However, if the fee is more than 1,500 yen to disclose due to video footage, etc., then any extra expenses will be charged and must be paid before a response is given. This fee is non-refundable even if we decide not to issue a disclosure or we do not have the data.
* No fee applies when applying for a correction.
