To get a mobile phone, you need to prepare identification documents.
Please prepare one of the following types/sets of documents requested. (Photocopies are not accepted.)

For an Individual Contract

  • Japanese Driver's License
  • Basic resident registeration card
  • Japanese Health Insurance Card
  • Student ID
    + Japanese Health Insurance Card or Certificate of residence or Certificate of Alien Registration
  • Credit card
    +Japanese Health Insurance Card or Certificate of residence or Certificate of Alien Registration
  • Certificate of Alien Registration
    + Passport
  • Certificate for Persons with Physical Disabilities or the Mentally Retarded or the Mentally Disabled

For a Corporate Contract

  • Certified Copy of Commercial Registration(Tokiboto(sho)hon)(certificate of present matter /historical records) or Certificate of registered seal.
    + Corporate applicantfs proof of identity
    + Employee ID
